Process Disconnects

What are process disconnects?

Simply put, a process disconnect is the point at which one process should have connected with another, but didn't. This can be as a result of someone not doing what they should have done, or something not working properly. Process disconnects can be characterized by:

Inputs not being received
Inputs being received at the wrong time
Inputs being received, but not as they were expected

Process disconnects are opportunities for process improvement, and they form one of the bases upon which continuous improvement initiatives can be built.
