Process Disconnects
What are process disconnects?
Simply put, a process disconnect is the point at which one
process should have connected with another, but didn't. This can be as a
result of someone not doing what they should have done, or something not
working properly. Process disconnects can be characterized by:

Inputs being received at the wrong time |

Inputs being received, but not as they were expected |
Process disconnects are opportunities for process improvement,
and they form one of the bases upon which continuous improvement initiatives
can be built.
